- Direct Deposit
- Used to set up a direct deposit of VA benefits. Use this form after notification from VA that you have been approved for benefits. Fax to (918) 781-7578 or go to www.godirect.gov and request on-line.
- Change of Address
- Used to notify VA of a change in address. This will change where the mail will be sent to. Fax to 612 970-5724.
- Due Process Waiver
- Used when the VA has proposed to rate the claim incompetent to handle their financial affairs. If the doctor indicates on the Physicians Report that the claimant is not able to handle their finances, then this for will be required.
- Third Party Disclosure
- Used to allow VA to talk to a third party, such as a son or daughter. Use one form for each person that you want the VA to talk to. Fax to (215) 381-3166. This form will not be submitted with the application.
