Co-Occurring Disorder System Transformation Project In January 2007 Placer County Health & Human Services began a new venture to develop the Co-Occurring Disorder System Transformation Project. Acknowledging that Co-Occurring Disorders in our client's is an expectation rather than exception and that historically these clients tend to have poorer outcomes with higher service utilization spurred the idea to adopt a new philosophical shift. Our system goal is to provide more welcoming, accessible, integrated, continuous and comprehensive services to individuals and families with health and/or substance use concerns. A committee of Change Agents, individuals identified that are committed to achieving this has been formed to identify gaps and improve our system. We will be working to educate and empower clients, organizations and our community to ensure integration of services and co-occurring competency.
