We work to meet the preventive health needs of families and individuals, and promote positive health behavior.

• Referrals for medical care and other resources
• Nutrition assessment and referrals
• New baby care and bonding
• Referrals for medical care and other resources
• Service coordination
• Health counseling
• Assessments and plans of care
• Assessing the family as a whole for needs, strengths, and resources
• High risk infant home visitation
• Wellness program referrals
• Breastfeeding and postpartum referrals and support
