The Placer County Vital Statistics Office provides copies of birth and death records for the current and previous year only.

The Placer County Clerk Recorders Office provides copies of prior year certificates

Applications & Fees

Due to the increase in cases of identity theft, there are two kinds of records that can be issued:

Authorized Copy

An “Authorized Copy” can be used to provide identity and is only given to someone authorized to receive it. A person authorized to get a certified copy of a birth or death record must be one of the following:

  • The registrant or a parent or legal guardian of the registrant.
  • A party entitled to receive the record as a result of a court order, or an attorney or a licensed adoption agency seeking the birth record in order to comply with the requirements of Section 3140 or 7603 of the Family Code.
  • A member of a law enforcement agency or a representative of another governmental agency, as provided by law, who is conducting official business.
  • A child, grandparent, grandchild, sibling, spouse, or domestic partner of the registrant.
  • An attorney representing the registrant's estate, or any person or agency empowered by statue or appointed by a court to act on behalf of the registrant or the registrant's estate.
  • A funeral director ordering certified copies of a death certificate on behalf of an individual specified in paragraphs (1) to (5), inclusive, of subdivision (a) of Section 7100 of the Health and Safety Code.

Informational Certified Copy

An “Informational Certified Copy” is for information only and cannot be used to prove identity. It will be labeled “INFORMATIONAL, NOT A VALID DOCUMENT TO ESTABLISH IDENTITY”

Two ways to obtain an Authorized or Informational Certified Copy:

In Person

Come to the Placer County Vital Statistics Office Monday-Friday 8:00am- 12:00pm or 1:00pm-4:15pm located at:

  • Complete the application provided at the office or use the links above under the “Applications & Fees” sections.
  • Sign a Sworn Statement in person declaring under penalty of perjury that you are a person authorized to receive a certified copy. For more information about authorized persons, please see "Authorized Copy" section above. No Sworn Statement is required for Informational Certified Copies.
  • Pay fees based on the records requested. Placer County accepts Cash or Check (Payable to PCVS). No ATM or Credit Cards Accepted.

By Mail

For Authorized Certified Copy

  • Obtain the correct form from the “Applications & Fees” section above.
  • Complete the form identifying you as the person authorized to receive a certified copy for purposes of identification.
  • Have you Sworn Statement notarized declaring you are authorized to receive the certified copy
  • Mail the notarized signed request and application with your payment (checks payable to PCVS) to:

Generally, you will receive your certified copy within two weeks by mail.

Note: Mail-in requests must be accompanied by a completed Sworn Statement (2 page of form) - signed in the presence of a Notary Public. Only one notarized Sworn Statement is required for multiple certificates requested at the same time; however, the Sworn Statement must include the name of each individual whose certificate you wish to obtain and your relationship to that individual.

For Informational Certified Copy

  • Obtain the correct form from the "Applications & Fees" section above.
  • A notarized signature is not required
  • If the form is unavailable, you may submit a written request with the full name, date of birth/death, and return address, along with the appropriate fees indicated above in the “Applications and Fees” section to:

Special Circumstances:

There are several different circumstances occur in which require amendments or changes to the original vital record. Click on any and all that apply to you for the instructions on how to amend your record. Please note that all applications for amendments require special forms; these forms can be obtained through the Placer County Vital Statistics office. Please call 530.889.7158 for more information and to request forms be mailed to you.
