- Complete the application provided at the office.
- Sign the Sworn Statement in person declaring under penalty of perjury that you are a person authorized to receive a certified copy. No Sworn Statement is required for Informational Certified Copies.
- Pay fees based on the records requested. Please note that the Placer County Vital Statistics office only accepts Cash or Check (Payable to PCVS). No ATM or Credit Cards Accepted.
