Diseases that Must Be Reported
- Clinicians are legally required to report Title 17 Diseases to the Placer County Health Department
- California Code of Regulations, Title 17, Section 2505 requires laboratories to report laboratory testing results suggestive of the Reportable Laboratory Conditions of public health importance to Placer County Health Department
Electronic Disease Reporting
Submit your Confidential Morbidity Reports (CMR) electronically through CalREDIE, California's web-based reporting system. To become a CalREDIE user, please complete and submit the CalREDIE User Registration Form and HIV/AIDS confidentiality Agreement. For more information, please call 530.889.7141

Note: Mailing reports delays mandatory quarantine protocols and can put others at risk of coming in contact with the animal. Rabies virus is transmitted through the saliva of a rabid animal when it bites a person or another animal. Any bite from a wild animal or unknown animal should be considered as a possible source of rabies.
Confidential Morbidity Report Forms
- Use the CMR-General Form for reporting all conditions except Tuberculosis and conditions reportable to the Department of Motor Vehicles (DMV)
- Use the CMR-TB Form to report Tuberculosis
- Use the CMR-LOC Form to report Lapses of Consciousness or Control, Alzheimer's Disease or other conditions which may impair the ability to operate a motor vehicle safely (pursuant to H&S103900). Please indicate specific disorder under "Condition being reported"
- Use the CMR-Animal Patients Form to report communicable diseases for animal patients
Reporting HIV/AIDS
Mailing Instructions for Reporting HIV/AIDS:
Double envelope all HIV/AIDS related materis
- Outer envelope (e.g. sender or recipient address or label) must have no references to HIV/AIDS
- Inner envelope will be marked "Confidential", sealed, and addressed to an authorized individual
- All mail must be sent by traceable courier services only, FedEX
- Electonic mail transmission (e-mail) of case informaiton containing personal identifiers is strictly prohibited, faxing this information is not recomended.

Reporting Outbreaks
Outbreaks must be reported to Placer County Health Department immediately
How to Report Gastrointestinal Outbreaks
1. Complete and submit the Initial GI Outbreak Report Form by faxing it to 530-886-2945
2. Complete and submit the GI Line List spreadsheet every 24 hours until all symptomatic cases resolve. Your CD investigator will instruct you where to send the line list
