Communicable Diseases Control Information

Information for Health Professionals

To report a communicable disease, save and complete the following form:  Confidential Morbidity Report (CMR) Form.   Fax the completed form to Placer County Communicable Disease Control at 530-886-2945.  If the disease requires immediate reporting, please call 530-889-7274 or toll free 888-822-7274.

Additional Information

One of Placer County's public health responsibilities is to help county residents protect themselves from contracting communicable diseases. None of the diseases mentioned here is common, but there are occasional cases. We provide this information so you can learn more about how to reduce the risk of contracting a communicable disease.

Because much of Placer County is still rural, residents have frequent opportunities to observe wildlife, some of which may carry communicable diseases. Also, many county residents enjoy outdoor activities, which can also lead to exposure to diseases.
