The blank forms on this page are intended to be printed first, then completed. Please note that you cannot submit any of these forms online. If you have problems or concerns about printing the forms from your computer, please contact our office and we can mail the forms to you. If you have questions regarding the completion and submission of the forms, please contact the Placer County Recorder's Office at 530-886-5600, Monday through Friday, between 8 a.m. and 5 p.m.

Common Clerk-Recorder Forms
- Birth Certificate Application (PDF) - Mail orders for an Authorized Birth Certificate must be notarized. Because of the increased occurrence of identity theft, new laws were passed by State legislators to protect birth and death records from misuse. For more information, visit the Vital Records page.
- Death Certificate Application (PDF) - Mail orders for an Authorized Death Certificate must be notarized. Because of the increased occurrence of identity theft, new laws were passed by State legislators to protect birth and death records from misuse. For more information, visit the Vital Records page.
- Confidential Marriage License - Both applicants must be present to apply for a marriage license. Please see our Marriage License page for more information.
- Cover Page with Notary Acknowledgement (PDF) - Required when there is not sufficient space for the recording information on the first page of a document. Recording fees are charged for this page. For more information, see Basic Recording Requirements.
- Fictitious Business Name (PDF) - For information about filing a Fictitious Business Name Statement, please see our Fictitious Business Name page.
- Preliminary Change of Ownership Report (PDF) (PCOR) - Required for any document that changes formal title to real property. The party receiving interest in the property or his/her attorney must sign the PCOR. If the required PCOR is not submitted or not signed by the correct party, an additional $20 fee will be due. Recording fees are not charged for the PCOR.
- Public Marriage License - English or Public Marriage License - Spanish. Both applicants must be present to apply for a marriage license. Please see our Marriage License page for more information.
