Vision benefits are provided by Vision Service Plan (VSP). Coverage is provided to employees at no cost and dependents can be added at the employee's expense. Dropped dependents must remain off the plan for one year. Visit the VSP website and either login or create an account to view your benefits information.
If you have a disabled dependent child, you may be able to continue vision benefits for them. You will need to fill out the disabled dependent certification form (PDF). For a child to be eligible as a disabled dependent, the following must be true:
The child is unmarried.
The child is incapable of self-support because of a mental or physical condition.
The child is over the age of 26.
The disability existed prior to reaching age 26 and continuously since age 26, as certified by a licensed physician.
