Medical Marijuana Identification Card Program (MMICP)

The Placer County Department of Health and Human Services - Public Health accepts applications for Medical Marijuana Identification Cards from qualified patients and caregivers. Do the following before submitting your application:

  • Obtain written documentation from your doctor for use of medical marijuana. The doctor can use the "Written Documentation of Patient's Medical Record," CDPH9044 (PDF) or CDPH9044 (Espanol) (PDF) or official letterhead. Ask your doctor to place a copy in your medical file.
